Anno 2019 heeft een massale ziekte vrijwel de hele wereldbevolking omgevormd tot vampiers. Eén minpuntje: de voorraad mensen is beperkt en door een drastisch bloedtekort beginnen de minderbedeelden onder de vampiers te transformeren in vleermuisachtige gedrochten.
Het is aan wetenschapper Edward (Ethan Hawke) om een bloedvervanger te fabriceren, maar wanneer hij in contact komt met een ondergrondse mensengroepering en een mogelijk genezingsmiddel voor vampirisme ontdekt, wordt ook hij een doelwit van zijn eigen organisatie.
